AKALAKA accepts referrals from North Carolina Tailored Care Managers serving members with the NC Innovations or Traumatic Brain Injury (TBI) Waiver. Individuals and families can also submit this referral for AKALAKA to contact the Care Manager to submit requests.

The North Carolina Traumatic Brain Injury (TBI) Waiver is for our community members who (1) are eligible for Medicaid based on their assets and income, (2) have residence in Durham, Wake, Cumberland or Johnston counties, (3) choose to participate in NC TBI Waiver services rather than institutional care, (4) have a need for NC TBI Waiver services specified in their person-centered Individual Support Plan, (5) use at least one non-respite NC TBI Waiver service monthly, (6) can "maintain health, safety, and well-being" in the community through budgeted NC TBI Waiver services, (7) live in a private residence or in a licensed facility with six or fewer persons unrelated to the owner of the facility, (8) need a level of care one may receive in a Skilled Nursing Facility (SNF) like a nursing home or a Specialized Hospital, (9) demonstrate the potential to benefit from specialized rehabilitation, and (10) require specialized brain injury services or supports that exceed SNF services.
Tailored Care Managers can submit requests to utilization management for individual, group, classes and conferences of the following Waiver services:
Community Networking for individuals with I/DD and/or TBI to connect with inclusive activities and pursue their personal goals.
Natural Supports Education for those that care about siblings with I/DD and/or TBI to receive support and resources to sustainably care for siblings as they pursue their personal goals.
AKALAKA is in-network with Vaya Health in the following counties:
Alamance, Alexander, Alleghany, Ashe, Avery, Buncombe, Caldwell, Caswell, Chatham, Cherokee, Clay, Franklin, Graham, Granville, Haywood, Henderson, Jackson, Macon, Madison, McDowell, Mitchell, Person, Polk, Rowan, Stokes, Swain, Transylvania, Vance, Watauga, Wilkes, and Yancey.
Any Local Management Entity/Managed Care Organization or Tailored Health Plan (Trillium Health Resources, Alliance Health, Eastpointe, Partners Health Management, and Sandhills Center), care management agency (CMA), or advanced medical home plus (AMH+) can also request invoices to bill Medicaid for AKALAKA home- and community-based classes and conferences, including in the following counties statewide in North Carolina:
